Forms are a great way to gather information from your customer. By creating your own form embedded directly on your page, you make it more convenient for your customer. They don't have to leave your Simplebooklet to complete your form.

The form builder has the following capabilities:

The ability to create multi-field forms that your audience can complete and submit right from your booklet page.
Dragging and dropping form creation with multiple form field styles.
Completed forms can be emailed to multiple recipients.
Completed forms are stored in the leads section of your Simplebooklet in analytics and on your dashboard under the Leads menu.
You can have multiple forms in a booklet.

Step-By-Step Guide



Below is a walkthrough showing you how you can build a form from scratch.

Open your Simplebooklet in the EDIT tab.

Click the Forms button from the toolbar on the left.

You'll arrive at a new tab with two options. Click Form Builder.

After clicking, you should then be directed to a new tab that will allow you to create your form by dragging form elements onto your form:
Header - Double tap to write in Label what you would like your header to say, and choose from the dropdown menu in Type what size you'd like it to be.
Paragraph - Double tap to write in Content what you would like your paragraph to consist of.
Text Field - Double tap to change the name of your Text Field in Label.
Text Area - Double tap to change the name of your Text Area in Label.
Select - Double tap to add and edit a dropdown menu in your form.
Radio Group - Double tap to change the name of your Radio Group in Label and edit Options to create multiple choice.
Checkbox Group - Double tap to change the name of your Radio Group in Label and edit Options to create checkboxes.
Date Field - Double tap to change the name of your Date Field in Label to allow your customers to input a date.
Number - Double tap to change the name of Number in Label to allow your customers to input numbers.
File Upload - Double tap to change the name of File Upload in Label to allow your customers to attach files.
Autocomplete - Double tap to change the name of Autocomplete in Label and input your answers in Options.
Hidden Input - Double tap to change the name of your Hidden Input in Label to allow your customers to customize a unique input.

After you're done designing your form, tap NEXT: ADD FORM RECEPIENTS.

You should then be directed to a new page that tells you your completed customized form will be sent to your account email address. You can change this email address in the space provided, or in ADD ANOTHER EMAIL.

When you're done, click ADD FORM TO PAGE.

Your form should then be automatically pasted directly in the middle of your Simplebooklet. Click to move and drag the corners to resize it.

You've successfully added and built a form on your Simplebooklet!
